CHRONIC CARE IMPROVEMENT PROGRAM
The Chronic Care Improvement Program (CCIP) is an intensive member education program designed to improve member's health and quality of life.
Viva Medicare offers a CCIP called "Viva's Healthy Heart Program" for beneficiaries who fall into the target population pertaining to cardiovascular disease. The Healthy Heart Program focus is to improve cardiovascular outcomes by promotion of the following:
- Avoidance of tobacco
- Blood pressure control
- Cholesterol control
- Exercise promotion
- Consideration of aspirin use
Viva's CCIP expects the target population to participate in self-monitoring that will result in improved blood pressure control, cholesterol control, increased physical activity and decreased readmissions. Like The 'Million Hearts Campaign', the clinical prevention focus is to optimize outcomes for populations that need treatment. Viva's CCIP program targets clinical prevention to the high risk and moderate risk members to optimize treatment.
